AB0169 INTEREST OF THE ECHOCARDIOGRAPHY IN SCREENING OF CARDIAC INVOLVEMENT IN RHEUMATOID ARTHRITIS PATIENTS

Background: Rheumatoid arthritis (RA) is the most common chronic inflammatory disease which usually affects peripheral joints.Extra-articular manifestations can occur during course of this and even before onset arthritis, such as cardiac impairment that a cause mortality in RA. Objectives: The aim our study to identify prevalence RA using echocardiography exam elaborate its associated factors. Methods: This cross-sectional including consecutive RA’s patients consulted rheumatology department Taher Sfar hospital. diagnosis was according ACR/EULAR 2010 criteria. epidemiological, clinical biological data were collected from patients’ records. Echocardiography performed by trained cardiologist same hospital, transthoracic approach. Results: Our included 67 patients: 63 women (94%) 4 men (6%), with an average age 52.55 years [21-75 years] mean duration 11.85 [10-40]. Joints deformities present 37 cases (55.2%) radiographic joint damage 49 (73.1%). 34 (50.7%) had ESR greater than 30 mm/hour 24 (35.8%) CRP 6mg/L. DAS28-CRP 3.35 [1.24-6.7] DAS28-ESR [1.4-7.35]. Cardiac symptomatology 22 (32.8%), dominated dyspnea on effort at rest chest pain 8 (11.9%)and palpitation 2 (3%). abnormalities found 44 cases. Valve detected 35 (52.2%), only significant. Valvular mitral valve regurgitation thickening 11 (16.4%) each, aortic 6 (9%),aortic (3%), stenosis 3 (4.5%),aortic calcification 12 (17. 9%) nodule 1 case (1.5%), pulmonary insufficiency 10 (14.9%) tricuspid 25 (37.3%).Pericarditis arterial hypertension (PAH) 5 (7,5%) Left ventricular hypertrophy 14 (20,9%). E/A E/E’ ratios revealed diastolic dysfunction 23 (34.3%).The longitudinal global strain measure -17,83 [-21,9 -10] it systolic 18 (26.86%). did not appear be (p=0.39) or (p=0.62) nor importance inflammation(p=0.1). However, joints activity (DAS28) significant predictors involvement (p=0.01; p=0.03). Conclusion: shows often asymptomatic correlated activity. represents excellent tool for detection early stages.
